Cialis (Tadalafil) was first approved for use in the U.S. in 2003, and since then it has been a popular medication for as-needed or daily use as a treatment for erectile dysfunction.
Cialis (Tadalafil) has a relatively good safety profile for use, in line with other medications in the same class, like Viagra (Sildenafil).
However, even though this medication has been around for nearly 20 years at this point, we do not currently have many very long-term studies looking into what happens if you end up taking Cialis (Tadalafil) for 5, 10, 20 years -- or maybe even longer.
Erectile dysfunction is not normally a condition that goes away after a short course of treatment. So, it makes sense that for most users, they will continue the medication as long as they are sexually active.
And, given that many men can start experiencing erectile dysfunction symptoms as early as their 30s or 40s, this might mean taking these medications for a very long time.
Below, we will cover just what we know about long-term Cialis (Tadalafil) usage, including that studies have been done on the subject, and what you should expect if you are planning on a long course of treatment.

There is currently no recommendation for stopping Cialis (Tadalafil) treatment after a certain amount of time. And, currently, we don’t see any new side effects developing with long-term use, outside of things that may have been happening from the start.
At the moment, it would be recommended to continue your Cialis (Tadalafil) treatment as long as your doctor deems it to be appropriate for you, and it still works for your ED symptoms.
Of course, if you are experiencing any new or ongoing side effects from treatment that are severe or are bothering you, it is important to let your doctor know in order to see if Cialis (Tadalafil) is still a good choice for you.
We don’t currently know about any new side effects that would develop from the long-term use of Cialis (Tadalafil).
If you are experiencing side effects from these medications, it is likely that you start noticing them when you first start using the medication or if you increase your dosage or usage.

Currently, the longest long-term safety study done looking into Cialis (Tadalafil) was completed in 2004. In this study 1173 men with erectile dysfunction who were taking 5mg, 10mg, or 20mg Tadalafil as needed (up to once daily) were followed up with for 24 months in order to see if there were any safety issues from long-term treatment.
The study concluded that the usage of these Tadalafil dosages as-needed (up to once daily) was safe and well-tolerated over the course of the 2-year trial. The researchers also go on to say that “These findings support the long-term use of tadalafil in the clinical management of erectile dysfunction.”
We still need more studies to conclusively see what happens with very long-term Cialis (Tadalafil) usage. But, the 2-year study that we have seems promising that long-term usage is likely a safe way to go about treating ED as long as is required by the patient, and deemed appropriate by a doctor.
It is also a good sign that this medication has been on the market for nearly 20 years (and likely has been used by some patients for potentially that same amount of time), and nothing striking has been noticed within the general public regarding issues from long-term use.
The very long-term studies for this medication are likely still forthcoming, but the current safety studies seem promising. Always take your medication as directed by your doctor and report any new or unusual side effects to a medical professional immediately.
